CBS Outreach

CBS Outreach is a ministry in which Church of the Blessed Sacrament provides direct assistance to the members of our community who are in need of financial assistance in obtaining dental and vision care.

Church of the Blessed Sacrament provides up to $125 of assistance for prescription medications, eye exams and glasses (not contacts), and dental work.  Clients must come in person and bring a picture ID and proof of residency in Tippecanoe County.  The client must bring a prescription or have a prescription waiting at the pharmacy.

1. Eyeglass World $125 Deal 

• Includes 
  • Eye Exam 
  • Two pairs of glasses
• Deal is only valid at Eyeglass World 
• Extras (tinting, unlined bifocals, etc.) can be purchased at client's expense

Requires two outreach visits.
Client may choose any optical business for exam and glasses.

• $49.99 at Eyeglass World, cost is higher anywhere else

• Client should get estimate for what he/she wants (lenses, frames)
• Client then makes a second Outreach appointment and brings estimate 
• Client will have $75.01 available (or $125 minus the cost of an eye exam some place other than Eyeglass world) for glasses purchase 
• Client may use available funds to purchase:
  • glasses
  • extras (tinting, unlined bifocals, etc.) 
  • protection plan
  • etc.
